may be helpful to obtain your medical record from your previous
doctor, if you have an extensive medical history. Fill out
this form and send it to your other doctor so that they can
release your medical record to us.
Payment
We accept most insurance plans and self-pay. Patients are responsible for their co-pays and deductibles. Your co-pay is due at the time of service. We accept cash, check, and credit cards.

We are on call 24 hours a day, seven days a week. Please call

Directions To The Office
The office is located next to Littleton Hospital, in the Arapahoe Medical Plaza III, building 7780, Suite 250.
From Broadway head east onto Mineral. Take your 2nd left. Look for the 1st building on your left, located at the southeast corner of the hospital campus.
